Cost wise it would depend on the time it takes to do which depends on whether there is any shading or very fine work etc Most tattoo places that I know of do not charge to get someones design ready for tattooing you just might have to wait a week or 3 depending on how busy the place is so that part should be free.
Pain is an individual thing - I have 1 arm band and 1 anklet and I have one on the centre upper back for me the most painful would have been the anklet - I thought the upper back would be really painful but after the first 5 minutes I was quite comfortable and happy to sit there for the 2 hours it took (mine has a lot of fine shading in it) and yet I have had other people tell me that their tattoo in exactly the same spot made them faint (hint go with food in your stomach and take along a sugary softdrink - it will reduce the risk of feeling dizzy and or fainting)
I have also had and done some body piercing (I did a course) and different people reacted differently to the same piercing - I suppose it depends on your pain threshold - have you already got any tattoos? If you do then you pretty much know what to expect if not I find the first 5 minutes a little painful and my way of getting through that is to concentrate on my breathing and try to think of only that - after that the endorphins and what ever else starts flowing and your skin seems to numb itself so that it really is quite bearable and comfortable.
Hope Ive helped :) Good luck with it and your own design will always mean more to you than 1 off the wall - my first 2 are off the wall and my last was a combination of 3 designs I found online and then manipulated into one image and added a few bits of my own and that one is my favorite one.

that is really sweet i wouldn't put any thing else around it and it might hurt a little more than your foot cause it is bigger but it isn't right on the bone

Not too bad. I have one on each shoulder blade and the only thing that hurts is about half way through the skin becomes very tender.

I have a celtic knot on my right shoulder blade.
it hurt the most on the bottom right.
it was my first tattoo and i always heard people say the outline was the worst but for me it was color. the lines are pretty close together and i could always tell when he was close to another line.
it definitly didnt hurt as bad as my other 2 which i have one on the back of my neck and one on the inside of my wrist...
as far as cost for your own design... i go to the same shop and same guys all the time and they will draw up whatever you want no charge. and if you bring in exactly what you want thats what you'll get

pain is relative and price is dependent on how long it takes etc
your 'design' wouldnt cost you any extra.

depending on the size of the tat cost can vary greatly...
for most larger tats I.E. back pieces and so on artist charge by the hour...
but for smaller they'll charge a set price... also nearly all shops have a Shop minimum... usually around $40 - $60 US...
shoulder blade isn't too bad... at least i didn't think so... but i have 6 tats... so maybe i'm weird...

Most tatoo artist charge by the hour, though soom charge by size. As for pain... I have one on my shoulder blade and it stung like all get out when they did the outline right on the blade. So it will probably hurt, at least a little.

my mum has one on her shoulder she said it hurt a little but not like my leg just got split open kinda hurt

Price depends on where you go. If you have some extra weight on you, then it can be less painful in that area. If you are thin and boney, then it will be more painful.
